A Subject Access Request (SAR) is a request under the Data Protection Act 2018, to an organisation asking for a copy of personal data that it holds about you.
An SAR enables you to understand how and why an organisation is using your data and to check that they are doing it lawfully.
How to make a request
You can make a request for your health records here by creating an account and logging into the portal.
Your portal account will allow you to view the request status, any information relating to your request, and enable you to download your requested information as soon as it is ready.
Please only complete ONE application form for any records you require.
Requirements
Before your request can be processed you will need to submit appropriate forms of ID / Authorisation to allow you access to health records.
